| REGISTRATION |
| Final registration for the symposium and accommodation
in the hotel will be done on the enclosed registration form of the congress
travel agency "Revelin"
at Icici. Payment for accommodation will be done in advance at giro account of "Revelin" i.e. at the welcome desk in cash or with a cheque on arrival. We kindly ask the participants to fill in the registration form so that we could have their data and could prepare their accreditation badge, vouchers for services, a sufficient number of the proceedings etc. |

| We especially ask them to send in or bring with them the copy of the bank-transfer (if they pay in advance with a transfer) because on the report of ZAP it is not written to whom the payment refers. |

| REGISTRATION FEE |
| The registration fee of 550 kn + VAT is paid at the giro account of the "Revelin" or at arrival during the accreditation. | ||
Paying the registration fee the participants will get the right to be accredited, to get the proceedings and vouchers for receptions and the social events. |
